You will not be able to compete in any rodeo until every item is completed and approved by the State Secretary, who will return it if not completed correctly.

2008-2009 Membership Application (2 parts) National and Oregon forms

______ Member must Sign and Date Front of All Applications

_______ BOTH Parents Sign and Date Front of All Applications

_______ Take Oregon Form to your school, Make sure appropriate personnel sign the form.

_______ Fill out in FULL, all lines of information on BOTH NHSRA and OHSRA forms.

Minor’s Release Form ***BOTH Parents and Member must sign and have signatures Notarized!!***

_______ All Signatures must be notarized! (Must have ALL signatures, or an explanation, see bottom of form)

If only one parent is signing, must note reason on signature line. (Example: DECEASED, DIVORCED AND FULL CUSTODY, ETC)

Report Card

_______ Include one copy of latest grading period report card. Incoming Freshman joining at beginning of year, before 1st high school grading period, send in 8th grade final report card)

3. One copy of latest grading period Report Card

4. Cashiers Check or Money Order in the Amount of  $145.00 For Membership made out to OHSRA.

5. Mail all forms to
